Admission Process

We are pleased with your interest in Meadows Mennonite Home
and hope the information we are sharing will help to acquaint you
with the services and programs Meadows has to offer residents
and family members.

Meadows Home residents live in the unit that most closely fits their
personal needs for care. It is most beneficial to be able to talk with
you in person about the care needs of the loved one you are
inquiring about, so that you have a clear picture of the services,
as well as the costs to provide the care needed.

       

We appreciate you applying
for entrance into Meadows
Mennonite Home. There is
an application for shelter
care, Alzheimer care, and
nursing care. From this
application, we will be
gathering information about
the potential resident,
including:

family history
names
phone numbers of doctors
phone numbers of services
    received
social security
Medicare numbers
contact information
physical information
Insurance information
financial information
personal interests


We use the physical information to help us determine if we have the appropriate bed available.

Meadows accepts the following forms of payment:

Private pay
Medicaid
Medicare
